Responsibilities
- Patient Care: Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting with dignity and respect.
- Vital Signs: Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, reporting any abnormalities to the nursing staff.
- Hygiene & Comfort: Ensure patients maintain proper hygiene and comfort levels throughout the shift, including changing linens and repositioning patients to prevent bedsores.
- Meal Assistance: Serve meals according to dietary restrictions and assist patients with eating when necessary.
- Documentation: Accurately document patient care activities, behaviors, and changes in condition in electronic health records (EHR).
- Communication: Effectively communicate with residents, families, and the healthcare team to ensure a continuum of care.
Qualifications
- Licensure: Current and valid Wisconsin State CNA License.
- Certifications: Current Basic Life Support (BLS) and CPR certification.
- Experience: Prior experience in a long-term care or home health setting is preferred.
- Physical Stamina: Ability to stand, walk, and lift up to 50 lbs frequently throughout the shift.
- Communication: Excellent verbal and written communication skills.
