Responsibilities
- Provide compassionate in-person patient care in Sacramento-area facilities and residences
- Assist with daily living activities including bathing, dressing, and mobility
- Monitor vital signs and document patient health data via our mobile health platform
- Coordinate telehealth appointments and remote medication reminders
- Collaborate with RNs and care teams through our virtual care hub
- Maintain accurate electronic health records using our cloud-based system
Qualifications
- Active California CNA certification in good standing
- Minimum 6 months of recent clinical experience
- Basic proficiency with smartphones/tablets for remote documentation
- Valid CPR certification (or willingness to obtain)
- Ability to work flexible shifts including evenings/weekends
- Clear TB test and background check
- Reliable transportation for in-home visits
