Responsibilities
- Provide Direct Patient Care: Assist patients with daily living activities, including bathing, dressing, and mobility support in home or facility settings.
- Monitor Vital Signs: Accurately track and record patient temperature, blood pressure, pulse, and respiration rates.
- Administer Medication: Support patients with medication reminders and assist with medication administration under supervision.
- Document Care Plans: Maintain detailed and accurate electronic health records (EHR) documenting patient progress and care provided.
- Remote Communication: Serve as a point of contact for patient inquiries and coordinate with the wider healthcare team via secure telehealth platforms.
- Emergency Response: Respond promptly to patient emergency situations and follow established safety protocols.
Qualifications
- Valid License: Current and active California CNA License (CN 1) and CPR/BLS certification.
- Experience: Minimum of 1 year of experience in a clinical or home health setting.
