Responsibilities
- Provide direct personal care and assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and record patient vital signs, including blood pressure, temperature, pulse, and respiration rates.
- Assist patients with mobility, transfers, and wheelchair positioning to ensure safety and comfort.
- Prepare and serve meals according to prescribed dietary restrictions and individual preferences.
- Communicate effectively with patients, family members, and the nursing staff to ensure continuity of care.
- Maintain a clean, sanitized, and organized patient living environment.
- Administer medications and treatments as directed by the Registered Nurse (RN).
Qualifications
- Certified Nursing Assistant (CNA) license in good standing (New York State required).
- Current Basic Life Support (BLS) and CPR certification.
- High School Diploma or GED required.
- Previous experience in home health or long-term care is preferred.
- Strong interpersonal and verbal communication skills.
- Ability to perform physical duties including lifting, bending, and standing for extended periods.
- Reliable transportation and valid driver's license (for home health visits).
