Responsibilities
- Provide compassionate personal care assistance including bathing, dressing, and grooming
- Monitor vital signs and report changes in patient condition to RN
- Assist with mobility, transfers, and range-of-motion exercises
- Administer medications under RN supervision and document accurately
- Maintain clean, sanitized patient environments and equipment
- Support emotional well-being through companionship and active listening
- Collaborate with interdisciplinary care teams to develop care plans
Qualifications
- Active NYS CNA certification in good standing
- Minimum 1 year experience in long-term care or geriatric setting
- Current BLS/CPR certification
- Ability to lift 50+ pounds and stand for extended periods
- Strong communication skills and empathy for elderly patients
- Proficient in electronic health record documentation
- Flexibility to work evenings, weekends, and holidays
- NYS driver's license preferred for transportation duties
