Responsibilities
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and report changes in patients' physical or mental condition to the RN case manager.
- Prepare nutritious meals and assist with meal planning based on specific dietary restrictions.
- Provide essential companionship and emotional support to patients and their families.
- Accurately document daily care logs and shift reports in compliance with company policies.
- Accompany patients to medical appointments, therapy sessions, or community outings.
- Perform light housekeeping duties to maintain a safe and clean living environment.
Qualifications
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required.
- Current CPR and Basic Life Support (BLS) certification.
- Valid California Driver's License and reliable personal transportation.
- Excellent communication skills and a genuine desire to help others.
- Ability to pass a background check and drug screening.
- Experience with elderly or disabled patients preferred.
