Responsibilities
- Provide high-quality patient assessment and care coordination via telehealth and in-clinic settings
- Administer medications and treatments per physician protocols
- Document patient encounters accurately in Epic EHR systems
- Collaborate with interdisciplinary teams for holistic care plans
- Monitor patient vitals and respond to emergent situations
- Educate patients on health management and medication adherence
- Participate in quality improvement initiatives
Qualifications
- Active Oregon RN license (compact license accepted)
- 2+ years clinical nursing experience
- BLS and ACLS certifications required
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and communication skills
- Ability to work independently in remote settings
- Epic EHR experience preferred
- Flexibility to work 20-24 hours/week across shifts
