Responsibilities
- Provide virtual patient assessments, triage, and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop personalized care plans
- Administer medications and treatments during in-person patient visits (as needed)
- Document clinical notes accurately in electronic health records
- Monitor patient progress and adjust interventions based on outcomes
- Educate patients on chronic disease management and preventive care
- Participate in quality improvement initiatives and telehealth program development
Qualifications
- Active RN license in Wisconsin (or compact state)
- Minimum 2 years clinical nursing experience
- BLS/CPR certification required
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- Strong assessment and critical thinking skills
- Excellent written and verbal communication abilities
- Ability to travel to patient locations within Milwaukee metro area
- Proficiency with EHR systems (e.g., Epic, Cerner)
