Responsibilities
- Conduct virtual patient assessments via telehealth platform
- Monitor and document patient vitals remotely using digital tools
- Provide health education and medication reminders via phone/video
- Coordinate care plans with RNs and physicians
- Assist with insurance verification and administrative tasks
- Maintain HIPAA-compliant electronic health records
- Respond to patient inquiries within specified SLAs
Qualifications
- Active Texas CNA certification
- 1+ years clinical experience preferred
- Strong computer literacy with EHR systems
- Excellent verbal/written communication skills
- Ability to work independently with minimal supervision
