Responsibilities
- Deliver remote patient assessments via telehealth platforms
- Provide clinical guidance and health education to patients
- Manage electronic health records with precision and confidentiality
- Collaborate with interdisciplinary care teams virtually
- Monitor patient progress and adjust care plans accordingly
- Respond to urgent patient inquiries within SLA timeframes
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Minnesota
- 2+ years clinical nursing experience
- BLS certification required
- Proficiency with telehealth technology (Zoom, Doxy, etc.)
- Exceptional communication and critical thinking skills
- Ability to work independently with minimal supervision
- Experience in case management or chronic disease preferred
- Commitment to HIPAA compliance and data security
