Responsibilities
- Provide virtual patient assessments and triage services via telehealth platform
- Develop and implement personalized care plans for diverse patient populations
- Coordinate care with multidisciplinary teams including physicians and specialists
- Document patient encounters accurately in EHR systems with HIPAA compliance
- Educate patients on chronic disease management and medication protocols
- Monitor patient progress and adjust interventions as needed
- Participate in peer review and quality improvement initiatives
- Respond to after-hours patient inquiries within service level agreements
Qualifications
- Active Florida RN license with no disciplinary actions
- Minimum 2 years clinical experience (telehealth preferred)
- BLS/CPR certification and current DEA license
- Proficiency in EHR systems (Epic, Cerner, or similar)
- Exceptional communication and virtual assessment skills
- Ability to work independently with minimal supervision
- Experience with chronic disease management (diabetes, hypertension, etc.)
- Strong problem-solving abilities in remote care settings
