Responsibilities
- Provide comprehensive telehealth assessments and patient consultations via secure video platform
- Monitor patient vitals and medication adherence through digital health records
- Collaborate with interdisciplinary teams to develop personalized care plans
- Document clinical encounters accurately in EHR systems
- Conduct virtual triage for acute and chronic care management
- Participate in quality improvement initiatives for remote care delivery
- Maintain HIPAA compliance in all digital communications
Qualifications
- Active RN license in Texas (TX)
- BSN degree or equivalent experience
- 2+ years clinical nursing experience
- Telehealth certification or willingness to obtain
- Proficient with EHR systems (Epic preferred)
- Strong critical thinking and decision-making skills
- Exceptional communication and empathy
- Ability to work independently with minimal supervision
