Responsibilities
- Deliver high-quality telehealth consultations and patient assessments via secure virtual platforms
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Monitor patient progress and provide timely interventions for chronic condition management
- Document patient encounters accurately in electronic health records (EHR) systems
- Provide health education and coaching to patients and their families
- Respond to urgent patient inquiries and triage appropriately
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Minnesota (or compact state)
- 2+ years of clinical nursing experience (telehealth preferred)
- BSN or higher degree strongly preferred
- Proficiency with telehealth platforms and EHR systems
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Valid BLS certification
