Responsibilities
- Conduct virtual patient assessments and develop personalized care plans
- Provide telehealth consultations for chronic disease management
- Coordinate care between patients, providers, and community resources
- Document clinical encounters accurately in EMR systems
- Educate patients on medication adherence and health maintenance
- Collaborate with interdisciplinary care teams
- Monitor patient outcomes and adjust interventions as needed
Qualifications
- Active RN license in Colorado (compact license accepted)
- Minimum 2 years clinical experience
- Telehealth or virtual care experience preferred
- Strong proficiency with EMR systems and telehealth platforms
- Excellent communication and critical thinking skills
- BLS certification required
- Ability to work independently and manage time effectively
