Responsibilities
- Provide high-quality virtual patient assessments via telehealth platform
- Develop and implement individualized care plans remotely
- Coordinate with multidisciplinary teams for seamless patient transitions
- Document clinical encounters accurately in EHR systems
- Educate patients on chronic disease management and medication protocols

Qualifications
- Active RN license in Colorado (Compact License preferred)
- Minimum 2 years clinical experience in acute care or telehealth
- BLS/ACLS certification required
