Responsibilities
- Perform rapid patient assessments via telehealth and in-person triage
- Collaborate with physicians to prioritize patient cases based on acuity
- Document clinical encounters accurately in Epic EMR systems
- Provide clear patient education on home care instructions
- Coordinate care transitions between departments and facilities
- Maintain strict compliance with HIPAA and TJC standards
- Participate in continuous quality improvement initiatives
Qualifications
- Current CA RN license (required)
- Minimum 2 years ER/urgent care experience
- BLS and ACLS certifications (maintained)
- Proficient in telehealth triage protocols
- Exceptional critical thinking and communication skills
- Epic EMR system proficiency
- Ability to work independently in fast-paced environments
- Spanish bilingual preferred (not required)
