Responsibilities
- Conduct comprehensive telephone triage assessments to determine patient acuity and urgency of medical needs.
- Utilize evidence-based protocols to recommend appropriate care pathways, including home remedies, urgent care visits, or emergency interventions.
- Document all patient interactions, clinical findings, and care plans accurately within the EMR system.
- Coordinate referrals to specialists and manage patient follow-up appointments efficiently.
- Maintain strict patient confidentiality and adhere to all HIPAA regulations and company policies.
- Collaborate with physicians and nursing staff to resolve complex patient care issues and escalations.
- Provide educational guidance to patients regarding disease prevention and health maintenance.
Qualifications
- Active, unencumbered Kentucky Registered Nurse (RN) license.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Minimum of 2 years of clinical nursing experience, with at least 1 year specifically in triage or telehealth.
- Strong verbal communication skills with the ability to de-escalate sensitive situations and build rapport.
- Proficiency in electronic medical record (EMR) systems (e.g., Epic, Cerner).
- Ability to work independently and make critical decisions under pressure while maintaining composure.
