Responsibilities
- Conduct comprehensive symptom assessments via phone and secure messaging platforms to triage patient needs effectively.
- Analyze patient history and current complaints to determine appropriate level of care and urgency.
- Coordinate timely referrals to physicians, specialists, or emergency services when clinical necessity arises.
- Provide detailed patient education on treatment plans, medication management, and preventative health measures.
- Maintain accurate and up-to-date electronic health records (EHR) while strictly adhering to HIPAA regulations.
- Collaborate with the broader care team to ensure continuity of service and patient satisfaction.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the state of Louisiana.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Minimum of 2 years of clinical nursing experience, with at least 1 year specifically in triage or telehealth preferred.
- Strong critical thinking, decision-making skills, and ability to remain calm under pressure.
- Excellent verbal and written communication skills with a patient-centric approach.
