Responsibilities
- Deliver remote patient monitoring and telehealth consultations
- Perform clinical assessments in Boston-area partner facilities
- Develop and implement personalized care plans

Qualifications
- Active RN license in Massachusetts
- BSN or Associate's degree with 2+ years clinical experience
- Telehealth or remote care certification preferred

- Strong critical thinking and decision-making skills
- Ability to work independently with minimal supervision
- Flexibility to travel within Boston metro area
