Responsibilities
- Provide comprehensive telehealth assessments and patient education via secure video platforms
- Monitor and coordinate patient care plans using our proprietary EHR system
- Collaborate with in-person care teams for complex case management
- Lead virtual wellness programs for chronic disease management
- Train and mentor junior nurses on remote care protocols
- Participate in quality improvement initiatives for digital health services
Qualifications
- Active RN license in Massachusetts with 3+ years clinical experience
- Telehealth certification or equivalent digital health training
- Proficiency in Epic EHR and virtual care platforms
- BLS/ACLS certification and ongoing CE requirements
- Exceptional communication skills for virtual patient interactions
- Experience in acute care or specialty nursing preferred
