Responsibilities
- Deliver high-quality patient assessments and care coordination via telehealth platforms
- Collaborate with multidisciplinary teams to develop and implement personalized care plans
- Monitor patient progress and adjust interventions remotely using EHR systems
- Provide health education and support to patients and families through virtual channels
- Adhere to HIPAA compliance and hospital protocols for remote care delivery
- Participate in continuous quality improvement initiatives for telehealth services
- Maintain accurate documentation in electronic health records
Qualifications
- Active RN license in Maryland (compact license preferred)
- Bachelor of Science in Nursing (BSN) or higher
- 2+ years of clinical nursing experience in acute care or specialty area
- Proficiency with telehealth platforms and EHR systems (Epic preferred)
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
- Strong critical thinking and problem-solving abilities
- Excellent communication skills for virtual patient interactions
- Ability to work independently with minimal on-site supervision
