Responsibilities
- Deliver high-quality virtual patient assessments via telehealth platform
- Coordinate care plans with multidisciplinary teams
- Monitor chronic conditions and medication adherence remotely
- Provide patient education and health coaching via video/phone
- Document encounters accurately in EHR systems
- Respond to urgent patient inquiries within 2-hour SLA
- Collaborate with on-site RNs for complex cases
- Participate in quality improvement initiatives
Qualifications
- Active Florida RN license (compact license accepted)
- 2+ years clinical nursing experience
- Telehealth certification or willingness to obtain
- Proficiency with EHR systems (Epic, Cerner preferred)
- Strong critical thinking and decision-making skills
- Exceptional communication and empathy
- Ability to work independently in remote environment
- BLS/CPR certification required
