Responsibilities
- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Provide direct patient care, including assistance with Activities of Daily Living (ADLs) and hygiene.
- Monitor and record vital signs, symptoms, and changes in patient condition accurately.
- Administer prescribed medications and treatments according to established protocols.
- Communicate effectively with patients, families, and medical staff to ensure continuity of care.
- Document all patient interactions and care provided in electronic health records.
- Maintain a clean, safe, and therapeutic environment for all patients.
Qualifications
- Current, unencumbered Registered Nurse (RN) license issued by the Massachusetts Board of Registration in Nursing.
- Graduation from an accredited Nursing Program (Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)).
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification preferred.
- Strong clinical assessment skills and attention to detail.
- Excellent verbal and written communication skills.
- Ability to work flexible hours, including nights, weekends, and holidays.
- Willingness to travel within the greater Boston area for home care assignments.
