- Conduct remote patient assessments and monitor vital signs via telehealth platforms.
- Document patient history, symptoms, and treatment plans accurately in electronic health records (EHR).
- Collaborate with physicians and healthcare teams to ensure optimal patient outcomes.
- Provide patient education and medication management guidance via phone and secure messaging.
- Identify potential health risks and escalate critical issues immediately.
- Maintain strict confidentiality and adhere to HIPAA compliance standards.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of Illinois.
- Graduation from an accredited nursing program (ADN, BSN, or ASN).
- CPR/BLS certification (American Heart Association).
- Strong computer proficiency and experience with remote communication tools.
- Excellent communication and interpersonal skills.
- Ability to work independently in a remote environment.
