Responsibilities
- Deliver remote patient assessments and consultations via telehealth platform
- Monitor and manage chronic conditions through virtual monitoring systems
- Coordinate care plans with in-person clinical teams and specialists
- Document patient interactions accurately in our HIPAA-compliant EHR system
- Provide health education and crisis intervention via secure video channels

Qualifications
- Active RN license in Ohio (or compact state)
- Minimum 2 years clinical nursing experience
- Telehealth or remote care experience preferred

- Excellent written communication abilities
- BLS certification required
