What You Will Do:
Responsibilities
- Conduct comprehensive remote patient assessments and health evaluations via secure video conferencing and phone.
- Manage patient care plans, administer medications (if applicable in telehealth scope), and provide evidence-based nursing interventions.
- Document patient encounters accurately and in compliance with HIPAA and state regulations using Electronic Health Records (EHR).
- Collaborate closely with physicians, advanced practice providers, and care teams to ensure continuity of care and treatment adherence.
- Provide patient education and counseling on disease prevention, medication management, and lifestyle modifications.
- Monitor patient symptoms remotely and escalate urgent medical concerns to appropriate providers immediately.
- Maintain a high standard of clinical excellence and customer service in every virtual interaction.
Qualifications
- Current and active Registered Nurse (RN) license issued by the State of Florida (FL).
- Minimum of 2 years of nursing experience (Telehealth, Case Management, or Acute Care preferred).
- BSN or Associate Degree in Nursing (ADN) required; Master’s degree preferred.
- Strong computer literacy and proficiency with EMR/EHR systems.
- Excellent verbal and written communication skills with the ability to build rapport remotely.
- Ability to work independently in a remote environment with strict adherence to safety protocols.
- CPR/BLS certification (ACLS preferred).
