Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments in accordance with state regulations and provider orders.
- Monitor vital signs and patient progress, escalating concerns immediately to the healthcare team.
- Communicate effectively with patients, families, and interdisciplinary healthcare professionals.
- Document patient encounters, interventions, and outcomes accurately in the EMR system.
- Provide patient education and health coaching to promote self-management and wellness.
- Participate in quality improvement initiatives and clinical audits.
Qualifications
- Current and active RN license in the State of Ohio.
- BLS (Basic Life Support) and ACLS certifications required.
- Minimum of 1-2 years of nursing experience preferred.
- Strong proficiency with Electronic Health Records (EHR) and telehealth platforms.
- Excellent verbal and written communication skills.
- Ability to work independently in a remote setting while maintaining high standards of patient safety.
- Compassionate demeanor with a focus on patient-centered care.
