Responsibilities
- Conduct virtual patient assessments via telehealth platform
- Develop and implement individualized care plans
- Monitor chronic conditions and adjust treatment protocols
- Coordinate with in-person providers for seamless care transitions

Qualifications
- Active RN license in Indiana (IN)
