Responsibilities
- Provide direct patient care, including bathing, dressing, and feeding assistance, to ensure the highest quality of life.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse, accurately and in a timely manner.
- Assist with mobility, transfers, and ambulation using proper body mechanics to prevent injury.
- Communicate changes in patient conditions to the RN or Physician immediately.
- Administer medications as prescribed and document all treatments in electronic health records (EHR).
- Maintain a clean, safe, and organized patient environment in accordance with infection control protocols.
- Build trusting relationships with patients and families, providing emotional support and education.
Qualifications
- Current and valid CNA or LVN license in the State of California.
- Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification required.
- Previous experience in a hospital, nursing home, or home health setting is highly preferred.
- Strong communication skills and the ability to work effectively within a multidisciplinary team.
- Demonstrated ability to handle sensitive patient information with strict adherence to HIPAA guidelines.
- Physical stamina to stand for long periods and lift patients as needed.
- Positive attitude and a genuine passion for patient care.
