Responsibilities
- Conduct comprehensive telehealth patient assessments and health evaluations.
- Develop and implement individualized care plans for patients in the Fort Worth region.
- Coordinate care transitions between facilities and home settings.
- Maintain accurate, timely documentation in electronic health records (EHR).
- Collaborate with physicians, social workers, and interdisciplinary teams.
- Provide patient education and support via phone and secure messaging platforms.
Qualifications
- Active, unencumbered RN License in the State of Texas.
- Minimum of 2 years of nursing experience (Case Management or Home Health preferred).
- Strong proficiency with electronic health records (Epic, Cerner, or similar).
- Excellent verbal and written communication skills.
- Self-motivated with the ability to work independently in a remote environment.
- BLS and ACLS certifications required.
