Responsibilities
- Conduct comprehensive phone and video triage assessments to evaluate patient symptoms and medical history accurately.
- Utilize clinical judgment to determine the acuity of the patient's condition and recommend the appropriate level of care or referral.
- Provide clear, concise health education and instructions regarding medications, treatment plans, and self-care.
- Document all triage encounters thoroughly and accurately in the EMR system to ensure continuity of care.
- Collaborate effectively with physicians, nurse practitioners, and care coordinators to ensure seamless patient transitions.
- Manage high call volumes with composure while maintaining a high standard of patient service.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Minnesota required.
- Minimum of 2 years of nursing experience, with at least 1 year in a triage or telehealth setting strongly preferred.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong verbal communication skills and the ability to remain calm and professional under pressure.
- Proficiency in EMR/EHR systems (Epic, Cerner, AthenaHealth) is highly desirable.
- Must possess a computer and high-speed internet connection for remote work.
