Responsibilities
- Conduct Comprehensive Assessments: Utilize evidence-based protocols to evaluate patient symptoms via phone, email, or secure messaging.
- Triage & Prioritization: Accurately assess urgency and prioritize patient needs to ensure critical cases are seen immediately.
- Documentation: Maintain accurate, timely, and detailed electronic health records (EHR) for all patient interactions.
- Provider Collaboration: Communicate patient findings and recommendations effectively to physicians and other members of the care team.
- Education: Provide patient education on health maintenance, disease prevention, and medication management.
- Policy Adherence: Ensure all clinical practices comply with HIPAA regulations and facility protocols.
- Issue Resolution: Assist in resolving patient inquiries and escalating complex cases to appropriate resources.
Qualifications
- Current WI RN License: Must hold a valid and active Registered Nurse license in Wisconsin.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 2 years of recent clinical nursing experience, with a preference for prior triage or telehealth experience.
- Communication Skills: Excellent verbal and written communication skills with a focus on patient empathy.
- Technical Proficiency: Comfortable navigating electronic medical records (EHR) systems.
- Problem Solving: Strong critical thinking and decision-making abilities in high-pressure situations.
- Reliability: Ability to work rotating shifts, including evenings, nights, and weekends as required.
