Responsibilities
- Conduct virtual patient assessments via telehealth platform
- Provide clinical guidance and education for chronic disease management
- Coordinate care transitions and collaborate with multidisciplinary teams
- Document patient encounters accurately in EMR systems
- Triage urgent cases and escalate to providers as needed
- Monitor patient vitals and medication adherence remotely
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Illinois
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- Proficiency with EMR systems and telehealth platforms
- Strong critical thinking and assessment skills
- Excellent communication and interpersonal abilities
- BLS certification required
- Ability to work independently with minimal supervision
