Responsibilities
- Conduct comprehensive patient assessments and evaluations via phone and secure messaging channels.
- Apply clinical judgment to prioritize patient needs and recommend appropriate levels of care or emergency services.
- Document all triage encounters accurately and in compliance with HIPAA and state regulations.
- Collaborate with physicians, nurse practitioners, and care teams to ensure seamless continuity of care.
- Educate patients and families on disease management, preventative care, and medication adherence.
- Maintain up-to-date knowledge of clinical guidelines, telehealth best practices, and local healthcare resources.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Georgia.
- Minimum of 2-3 years of experience in acute care, triage, or emergency nursing preferred.
- Certifications: BLS (Basic Life Support), ACLS (Advanced Cardiovascular Life Support), and TNCC required.
- Strong computer proficiency with EMR systems (e.g., Epic, Cerner) and telehealth platforms.
- Excellent verbal and written communication skills with a focus on patient-centric care.
- Ability to remain composed and make critical decisions under pressure.
