Responsibilities
- Conduct comprehensive patient assessments via phone and secure portal to determine the acuity of symptoms.
- Provide medical triage advice and recommend appropriate care pathways, including home care, office visits, or emergency services.
- Facilitate patient referrals to appropriate specialists and healthcare providers within our network.
- Maintain accurate and timely documentation of all patient interactions and clinical findings in the EMR system.
- Collaborate with physicians and care teams to ensure continuity of care and optimal patient outcomes.
- Manage incoming high-volume inquiries with efficiency, empathy, and professionalism.
Qualifications
- Current, unencumbered RN license to practice in the state of Arizona.
- Minimum of 2-3 years of experience in a triage or clinical nursing setting.
- BLS and ACLS certification required (ACLS preferred).
- Strong clinical assessment skills and ability to make sound medical decisions under pressure.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Valid driver’s license and reliable transportation (as travel to satellite offices may be required).
