Responsibilities
- Conduct virtual health assessments and patient screenings via telehealth platform
- Provide medication reminders and health education remotely
- Coordinate care plans with registered nurses and physicians
- Monitor vital signs and report abnormalities to clinical teams
- Assist patients with scheduling telehealth appointments
- Maintain accurate electronic health records (EHR)
- Offer emotional support to patients and families during virtual sessions
Qualifications
- Active CNA certification in the state of Ohio
- Minimum 1 year of clinical experience (in-person or remote)
- Proficiency with telehealth software and EHR systems
- Strong communication skills and emotional intelligence
- Ability to work independently with minimal supervision
- Basic computer literacy and reliable high-speed internet
- Valid CPR certification (or willingness to obtain)
