- Coordinate care plans and communicate patient status to RNs and physicians
- Provide remote patient education on medication management and chronic conditions
- Document clinical encounters accurately in electronic health records (EHR)
- Advocate for patients and resolve care coordination challenges proactively
- Collaborate with social workers to address patient social determinants of health
- Participate in virtual interdisciplinary case conferences
Qualifications
- Active California CNA certification in good standing
- Minimum 2 years of clinical experience in healthcare setting
- Proficiency with telehealth software and EHR systems
