Responsibilities
- Provide virtual patient monitoring and telehealth support via secure platforms
- Conduct health assessments and document findings in EHR systems
- Coordinate care plans with multidisciplinary healthcare teams
- Deliver medication reminders and health education remotely
- Triage patient inquiries and escalate urgent issues appropriately
- Assist with scheduling virtual appointments and care coordination
- Maintain strict HIPAA compliance in all digital interactions
Qualifications
- Active NYS CNA certification in good standing
- Minimum 1 year clinical experience (telehealth preferred)
- Proficiency with telehealth platforms and EHR systems
- Exceptional communication and problem-solving skills
- Valid NY State driver's license and reliable internet
- Ability to work independently with minimal supervision
- CPR/BLS certification (current)
