Responsibilities
- Conduct virtual health assessments via telehealth platform
- Assist patients with medication management reminders
- Coordinate care plans with RNs and physicians
- Provide emotional support and patient education
- Document patient interactions accurately in EHR system
- Triage patient inquiries and escalate urgent concerns
- Collaborate with care teams to develop treatment plans
- Monitor patient progress and report changes
Qualifications
- Active CNA certification in good standing
- Minimum 1 year clinical experience
- Experience with telehealth platforms preferred
- Strong computer literacy and typing skills
- Excellent communication and empathy skills
- Ability to work independently with minimal supervision
- Valid CPR certification
- Pass background check and drug screening
