Responsibilities
- Provide high-quality remote patient care and monitoring under the supervision of a Registered Nurse.
- Conduct patient assessments and document vital signs and health status accurately.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Educate patients and families on disease management and health promotion strategies.
- Maintain strict confidentiality and compliance with HIPAA regulations and company policies.
- Utilize telehealth technology effectively to bridge the gap between clinical care and remote settings.
- Respond promptly to patient inquiries and escalate critical issues to nursing leadership.
Qualifications
- Current and valid Certified Nursing Assistant (CNA) license in the State of Texas.
- Previous experience in a healthcare setting (preferred for remote roles).
- Strong computer skills and proficiency with electronic health records (EHR) systems.
- Excellent communication skills, both verbal and written.
- Ability to work independently in a remote environment while maintaining high standards of patient care.
- Valid driver's license and reliable transportation (for occasional on-site duties if required).
- Empathy, patience, and a strong desire to help patients.
