Position Overview
As a Remote CNA, you'll provide exceptional patient care coordination through telehealth platforms and occasional in-person assessments in San Diego. This role combines remote flexibility with hands-on patient impact.
Responsibilities
- Monitor patient vitals and health status remotely via telehealth systems
- Coordinate care plans and communicate with patients/families
- Document patient information in electronic health records
- Assist with medication management and appointment scheduling
- Conduct periodic in-home health assessments in San Diego
- Provide health education to patients and caregivers
- Collaborate with healthcare teams for continuity of care
Qualifications
- Active CNA license in California
- Minimum 1 year CNA experience
- Telehealth platform proficiency
- Strong communication skills
- Ability to work independently
- Valid driver's license and reliable transportation
- Pass background check and drug screening
