Shift Details:
• 12-hour night shifts (7:00 PM - 7:00 AM)
• Rotating weekends

Responsibilities
- Conduct virtual patient assessments and health monitoring via telehealth platforms
- Coordinate care plans with on-site nursing staff and physicians
- Document patient observations and interventions in electronic health records
- Provide telehealth education to patients and family members
- Respond to emergency alerts and coordinate rapid response protocols
- Collaborate with interdisciplinary care teams for complex cases
- Maintain HIPAA compliance in all virtual communications
Qualifications
- Active California CNA certification required
- Minimum 1 year acute care or long-term experience
- Proficiency with telehealth platforms (Epic, Cerner, or similar)
- Strong critical thinking and decision-making skills
- Ability to work independently with minimal supervision
- Excellent written and verbal communication skills
- Valid California driver's license (occasional facility visits required)
