Responsibilities
- Monitor patient vitals and health status via telehealth platforms
- Coordinate care plans between patients, families, and clinical teams
- Document patient information accurately in electronic health records (EHR)
- Provide virtual health education and medication reminders
- Collaborate with RNs to ensure continuity of care
- Participate in virtual case conferences and training sessions
- Respond to patient inquiries through secure messaging systems
Qualifications
