Responsibilities
- Conduct virtual patient assessments and health monitoring via telehealth platform
- Coordinate care plans with RNs and interdisciplinary teams
- Provide medication reminders and health education remotely
- Document patient interactions accurately in EHR systems
- Respond to patient inquiries and triage urgent concerns
- Collaborate with on-field staff for in-home follow-up visits
- Maintain HIPAA compliance in all digital communications
Qualifications
- Active Texas CNA certification required
- 1+ years of clinical experience preferred (new graduates welcome)
