- Coordinate patient scheduling and virtual care coordination via HIPAA-compliant platforms
- Assist nurses with electronic health record documentation and data entry
- Conduct telehealth patient intake assessments and symptom screenings
- Facilitate virtual family education sessions for post-discharge care plans
- Manage medication reconciliation and supply inventory tracking
- Provide remote wellness check-ins for stable chronic condition patients
- Collaborate with interdisciplinary teams on care plan updates
Qualifications
- High school diploma or equivalent (no experience required)
