Responsibilities
- Coordinate patient care plans via our secure telehealth platform
- Conduct virtual wellness checks and medication reminders
- Assist patients with scheduling and appointment coordination
- Document patient interactions accurately in our EHR system
- Collaborate with clinical teams to ensure continuity of care
- Participate in ongoing training and professional development
- Maintain strict HIPAA compliance in all communications
Qualifications
- High school diploma or equivalent (GED accepted)
- No prior healthcare experience required
- Strong communication and empathy skills
- Basic computer proficiency (internet, email, video calls)
- Ability to pass background check and drug screening
- Must reside in Texas
- Willingness to obtain CNA certification (training provided)
