Responsibilities
- Provide compassionate in-home care including meal preparation, medication reminders, and personal hygiene assistance
- Monitor client health status and report changes to care coordinators via remote systems
- Transport clients to appointments and social activities using personal vehicle
- Maintain accurate care documentation through our secure digital platform
- Collaborate with family members and healthcare providers during virtual care conferences
- Create individualized care plans under remote supervision of RN Case Managers
- Implement safety protocols and emergency response procedures
Qualifications
- Minimum 1 year professional caregiving experience or CNA/HHA certification
- Valid Indiana driver's license with reliable transportation
- Proficient in telehealth platforms and digital care documentation tools
- CPR/First Aid certification (or willingness to obtain)
- Ability to pass comprehensive background check and drug screening
- Strong communication skills for remote client/family interactions
- Compassionate demeanor with patience and cultural sensitivity
- Flexible schedule availability including weekends and holidays
