Responsibilities
- Provide personalized in-home care including medication management, mobility assistance, and daily living activities
- Coordinate care plans via our digital platform for seamless remote communication with families and medical teams
- Monitor client health status and document observations using our telehealth tools
- Transport clients to appointments and community activities (mileage reimbursed)
- Assist with light housekeeping and meal preparation
- Build trusting relationships through empathetic communication and cultural sensitivity
Qualifications
- Minimum 1 year professional caregiving experience or CNA/HHA certification
- Valid Texas driver's license and reliable transportation
- Proficiency with smartphones/tablets for remote coordination tasks
- Ability to lift 50 lbs and perform physical care tasks
- Clear criminal background check and TB test results
- Compassionate demeanor with patience and strong problem-solving skills
- Flexibility to work weekends and some holidays
