Responsibilities
- Provide personalized care plans including medication management, meal preparation, and hygiene assistance
- Transport clients to medical appointments and community activities
- Monitor vital signs and document care progress via our digital platform
- Coordinate with healthcare providers and family members remotely
- Implement safety protocols and emergency response procedures
- Engage clients in therapeutic activities and social companionship
- Complete required documentation using our telehealth portal
Qualifications
- Minimum 1 year professional caregiving experience
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) preferred
- CPR/First Aid certification required (training provided)
- Valid Arizona driver's license and reliable vehicle
- Proficiency with telehealth platforms and digital documentation tools
- Exceptional communication skills for remote client interactions
- Ability to lift 50 pounds and perform physical care tasks
