Responsibilities
- Provide personalized care plans including medication management, meal prep, and mobility assistance
- Monitor vital signs and document health changes in our digital platform
- Coordinate telehealth appointments and family communication via our mobile app
- Transport clients to medical appointments and community activities
- Engage clients in cognitive stimulation and companionship activities
- Adhere to HIPAA compliance and infection control protocols
- Collaborate with healthcare professionals during care plan reviews
Qualifications
- Minimum 1 year professional caregiving experience
- Current CPR/First Aid certification
- Valid Arizona driver's license and insured vehicle
- Ability to lift 50 lbs and perform physical care tasks
- Proficient with telehealth technology and digital documentation
- Clear background check and TB screening
- Spanish bilingual preferred (not required)
- Compassionate demeanor with strong problem-solving skills
